htaccess.md package

Run `sh create-htaccess-md.sh` to create `htaccess.md` in the current directory.

To choose a target path: `sh create-htaccess-md.sh /path/to/htaccess.md`

The script only creates or overwrites the Markdown documentation file; it does not create or alter any `.htaccess` server-rule file.
